Acceptance and Commitment Therapy (ACT) was developed by Steven Hayes (e.g. Hayes at al., 2006) and is seen as a therapy model that does not treat the symptoms of individual psychological disorders, but rather teaches the patient skills to deal specifically with the symptoms and thereby promote your own behavioral flexibility and value-oriented actions in everyday life.
ACT therefore represents a prototypical procedure of the so-called “third wave” of behavioral therapy. Efficacy studies impressively show that these skills lead to a reduction in psychological symptoms without these always needing to be explicitly discussed (Powers et al., 2009).
For this reason, ACT can be used as a cross-disorder concept for secondary prevention in people who are already mentally ill or stressed. ACT attempts to balance the two essential dimensions of psychotherapy, “acceptance” and “change”: Numerous very intuitive exercises have been developed to promote tolerance even for higher intensity experiences of unpleasant emotions, to improve mindfulness for experiencing the moment, and to develop benevolent distance to become aware of one’s own thoughts and feelings, as well as one’s personal values, and to implement these in everyday life. Dialectical Behavioral Therapy (DBT) was originally developed as a disorder-specific treatment program for patients with borderline disorders and is recommended in the new S3 guidelines for borderline disorders in Germany as the most scientifically proven treatment method.
DBT is based on clear principles and rules that offer good orientation, especially in the case of complex disorders. Depending on the severity of the disorder, the focus of treatment is on teaching specific skills to cope with crises or on improving emotion regulation, self-concept and social interaction.
Methodologically, the DBT includes a wide field of evidence-based interventions that can be used on the basis of individual problem analysis.
In addition to the standard DBT for borderline disorders, a large number of adaptations to other disorders and different settings have been developed in recent years.
There are now 24 randomized, controlled studies in outpatient and inpatient settings that clearly demonstrate the effectiveness of DBT: DBT improves overall psychopathology, reduces the frequency of self-harm and inpatient stays. Social integration, work and partnership also return to normal. Today, DBT is considered the gold standard in the treatment of complex psychological disorders of emotion regulation. Accredited by and in close cooperation with Marsha Linehan and her team, we set ourselves the goal of spreading the DBT in German-speaking countries many years ago, which we have largely succeeded in doing.
